Age 3 Transition Resources

Parents of children nearing age 3 need to prepare for their child’s transition from early intervention to a public school district. Here’s who can help.

PENNSYLVANIA

The Intermediate Unit, a county-by-county educational entity, manages the transition in several ways. The IU:

Provides transition information 1 year before transition age, the I.U; sends letters telling you to begin thinking about transition and explains your options

Takes part in a transition planning meeting with you and your child’s Early Intervention provider

Introduces you to your local school district representatives.

Helps with paperwork, such as intent to register and other forms

NEW JERSEY

The Dept. of Health and Senior Services should have provided your child’s early intervention program. Prior to 120 days before your child’s 3rd birthday, a service coordinator from this department should assist in creating a plan to transition responsibility for your child’s education to your local school district.

DELAWARE

The Dept. of Health and Social Services manages early intervention and provides each child with an early intervention, or “Part C,” coordinator. Prior to your child reaching age 3, this coordinator is responsible for creating a smooth transition to your local school district. Your Part C coordinator will:

  Help develop a transition plan from E.I. to your local school district. The plan begins 9 months before your 3-year-old’s first day of school.

  Introduce you to your local school district contact

  Inform you about services to expect and your rights, and provides you with resources on developing an IEP (Individualized Educational Plan).

Help you give your child the necessary social skills before moving into a public preschool, if appropriate.
